I would like to know regarding the required documents proving decease, in case of succession or cancellation due to the death of the contract holder

Refer to below for examples of documents proving decease that must be submitted when proceeding for contract transfer or cancellation procedures due to the death of the contract holder (copy allowed).

Important note

Documents to verify death

  • Family Register Certificate (Koseki Touhon containing all records) or Extract of the Family Register Certificate (Koseki Shouhon containing individual record only)
    • One in which the death of the contract holder is recorded.

    • Issued within the last 3 months

  • Deletion Register Certificate (Joseki Touhon containing all records deletion) or Extract of the Deletion Register Certificate (Joseki Shouhon containing individual record deletion only)
    • One in which the death of the contract holder is recorded.

    • Issued within the last 3 months

  • Certificate of Residence (Juminhyo, excepting persons who have moved-out or are deceased)
    • One in which the death of the contract holder is recorded.

    • Issued within the last 3 months

If there is no mention of death in the above documents, please prepare one of the following separately. 

  • Certificate of Death
    • One that has been accepted by the local government (with acceptance stamp). However, if there is a note on the right side of the Hospital Death Record document, it will be accepted.

  • Hospital Death Record

  • Cremation (Burial) Permit

  • Condolence visit thanks Letter / Newspaper obituary column
    • One that has the full name of the deceased contract holder.

  • Return for condolence money
    • One that has the full name of the deceased contract holder.

  • Receipt of funeral hall usage fee
    • One that has the full name of the deceased contract holder.
  • All of the indicated amounts exclude tax unless otherwise specified.
