I want to know what documents are required for procedures by an agent

The documents required will differ depending on whether the visitor is other than a legal guardian or an adult guardian.

Please see below for the documents required for the procedure.

Documents required if the visitor is other than a legal guardian

Documents required for device upgrade

  • Current mobile phone (including IC card)

  • Identity verification document of the agent (in Japanese only)

  • Power of attorney from the contract holder

  • Identity verification document of the contract holder (copy allowed and only in case of installment payment contract) (in Japanese only)

  • Parental Consent Form and Filtering Service Application (if the contract holder is a minor)
  • If the contract holder is a minor and the representative is a facility-related person, a certificate verifying that the agent is a facility staff member is also required.

Documents required for cancellation

  • Current mobile phone (including IC card)

  • Identity verification document of the agent (in Japanese only)

  • Identity verification document of the contract holder (copy allowed and document to verify death in case of contract holder is deceased) (in Japanese only)

  • Power of attorney from the contract holder (a document that can prove the relationship when the contract holder is deceased)
  • If the contract holder is a minor and the representative is a facility-related person, a certificate verifying that the agent is a facility staff member is also required.

  • If the contract holder is an elderly person and the agent is the company of lifelong caregiving for the elderly, etc., a certificate verifying that the agent is an employee of the company is also required. In addition, a power of attorney is required even in the event of the contract holder's death, so it is necessary to prepare a power of attorney before the person dies.

Documents required for changing contract information

(Change of name or surname, address, contact phone number, current plan, optional services and discounts, change of user information)

  • Depending on the procedure, additional documents may be required.

Documents required for new contract or MNP

(Only if the contract holder is eligible for Smile Heart discount)

  • Identity verification document of the contract holder (in Japanese only)

  • Identity verification document of the agent (in Japanese only)

  • Smile Heart discount confirmation document of the contract holder

  • Set the payment method to one of the following

    In the case of credit card withdrawal: Credit card (including debit card)

    In the case of direct debit from bank account: Cash card or bank passbook + seal registered with the financial institution.

  • Parental Consent Form and Filtering Service Application (if the contract holder is a minor)

  • MNP reservation number (only for new MNP contracts)
  • If the contract holder is a minor and the representative is a facility-related person, a certificate verifying that the agent is a facility staff member is also required.

  • Identity verification document of the contract holder (copy allowed) (if the user is a minor and is not registered) (in Japanese only)

For your reference

  • If the visitor is other than an adult guardian or the contract holder is a minor and the visitor is a parent or guardian, the following documents will be accepted in place of a letter of attorney from the contract holder.

    In case the guardian/parent who has already notified our company: The identity verification document of the guardian with parental authority

    In case the guardian/parent who has not notified our company: A document that prove the parent-child relationship (health insurance card, etc.)

    • However, if the procedure for new contract, new MNP contract, device upgrade involves an application that requires the consent of the contract holder, a power of attorney from the contract holder is required.
  • All of the indicated amounts exclude tax unless otherwise specified.
